At MEDIROM MOTHER Labs, Inc. (hereinafter referred to as "the Company"), we conduct business related to health technology and specific health guidance. (2) Purpose of use in cases where personal information is acquired by a method other than writing directly from the person concerned

・For holding seminars for other companies contracted by the Company and the Medirom Group, conducting personnel and labor affairs, providing specific health guidance services, and providing various services related to the apps developed by the Company

・Recruitment applicant information from Hello Work and private job information sites is used for recruitment purposes

(3) Purpose of Use of Personal Information when Using Lav and MOTHER (including cases where information is acquired through Health Connect provided by Google LLC, upon consent of the service user)

※ We will collect the following information from users who provided consent to link their data with Health Connect: Health Connect user ID, weight, body fat percentage, step count, sleep records, and other relevant information. Please be aware that this information will be handled in compliance with the policies of Health Connect, including requirements for limited use.

In addition to the purposes of use outlined in the above (1) regarding customer information, the following purposes are included:

・Centralized management and display of body composition information (activity and health data obtained from designated devices such as movement distance, step count, sleep duration, weight, body composition, etc.), and other data provided by users themselves.

・Assistance in improving users' lifestyles.

・Providing counseling services, including dietary guidance based on body information such as gender, weight, and tailored diet and exercise recommendation, through channels such as WEB, face-to-face, telephone, email, etc.

・Utilizing the data for research and marketing activities, ensuring that individual identities are protected through anonymization or statistical processing.

・Supporting the implementation and planning of health measures of the relevant business operators as per the terms of business contracts.

    In regard to the personal data held by the Company, users have the right to request notification of the purpose of use, disclosure, correction, addition, or deletion of content, cessation of use, deletion, and cessation of provision to third parties (disclosure, etc.).

    Requests for disclosure, etc., of personal information will be handled according to the following procedure. Please note that all information recorded in Lav and MOTHER will be permanently deleted upon unsubscribing from these services. Once deleted, the information cannot be recovered.

    We will send you the "specified form" by mail. After its arrival, please agree to our "Handling of Personal Information" document, and send by mail or bring it in person with necessary materials to the following address.

    (2) Method of Confirming Whether the Requester for Disclosure, etc. is the Individual or a Representative.

    The Complaint and Consultation Officer shall confirm the identity of the person requesting disclosure, etc. by the documents specified below.

    To confirm that the requester for disclosure, etc. is a representative, in addition to the documents confirming the individual requesting disclosure, etc., the Complaints and Consultations Officer will verify the relationship between the individual requesting disclosure, etc., and the representative using the documents specified below, distinguishing between optional representatives and legal representatives (guardians of minors, adult guardians, parental authority).

    The documents for confirming the individual and the representative are required to be copies of identification cards issued by public authorities with photos, as exemplified below:

    ・Individual Number Card (front side)

    ・Residence Card or Special Permanent Resident Certificate

    The documents for proving the relationship between the individual subject to disclosure and the representative are as follows:

    ・For voluntary representatives: a written authorization stating that they have the authority to receive the results of disclosure, etc.

    ・For guardians of minors: a copy of the family register or a certificate of registered matters (both issued within six months from the date of issue).

    ・For guardians of an adult: a certificate of registered matters (issued within six months from the date of issue).

    ・For a parental authority: a copy of the family register or a resident certificate (both issued within six months from the date of issue).

    2. Fees for "Request for Disclosure, etc." and Collection Method

    Only in the case of requesting notification of purpose of use or disclosure, we will charge the following amount per request (including separate postage charges).

    Please enclose a postal fixed-amount money order of the amount below when sending the request documents.

    3. Notification of results

    The results of our internal review will be promptly communicated to you in the following manner.

    【 Provision of Personal Information to Third Parties 】

    MEDIROM MOTHER Labs, Inc. (hereinafter referred to as "the Company") will not provide users' personal information to third parties without their consent except in the following cases:
